How to Receive Your Rebate!
- Get a contact lens fitting and/or eye exam.
- Purchase required boxes as indicated above.
- Complete the mail-in certificate.
- Save your boxes and all receipts for rebate submission (photocopies are not acceptable),
for the qualifying purchases and a copy of your valid eye exam and/or contact lens fitting receipt.
- Go to cibavision-rebates.com and enter your Individual Rebate Code
- Download and complete the CIBA VISION® Official Rebate Form.
- If you don’t have access to a computer, call 877-886-4321
to obtain your rebate. This will add two weeks to processing time.
- Keep a copy of your paperwork for your records.
